Editing an assessment
From the assessments page, select the assessment number for the assessment you want to edit A summary of the patron's details are displayed at the top of the page that include the patron's ID, name, date of birth, exceptionalities, and the associated referral number.
Functionality
As well as being able to edit the assessment details, the following functions are available from the edit assessment page.
- Assigned Equipment
- Display equipment assigned to the patron, as well as equipment reserved by the patron, and the equipment usage history for the patron. Equipment may be assigned to the patron from this page.
- Related document links
- The documents associated with this patron.
- Add / Edit Training
- A training record may be added for this assessment if the "training needed" checkbox is checked in the training section of the assessment form.
Edit Assessments Form
The edit assessments form contains the following sections:
Assessment Details
The assessment details section contains the following fields:
- Service Provider
- A drop-down list containing a list of service providers set up in the configuration section.
- Assessment Date
- The date for the assessment.
- Observation date
- The date for the observation.
AT Permission Details
The AT permission details section contains the following fields:
- Permission Received
- A checkbox to indicate whether permission has been received to use AT.
- Training needed
- A checkbox to indicate whether training is required before the patron can use the AT.
Training Details
The training details section contains the following fields:
- Permission Date
- The date permission from granted for training.
- Report date
- Date the report is received.
- Persons to be trained
- The people that will receive the training on the AT.
Assessment Notes
Notes may be added to the assessment. Assessment notes are displayed at the end of the page in a data table including the date of the assessment, recommendations and the outcome from the assessment.
